That is enough to finish check-in without rushing. If you are running late, call — we would rather move you than have you drive across town for nothing.
Digital check-in
Forms are completed on a tablet, or in advance from home if you prefer. Health history is filled in with our team, not typed into a website.
Photos and scans you get to see
An intraoral camera and digital X-rays where they are needed. Everything goes on the screen beside the chair so you are looking at your own teeth, not a diagram.
A conversation, then a plan
Dr. Zamiri goes through what she found, what needs doing now, what can wait, and what she would simply keep an eye on. You ask questions until it makes sense.
An estimate before treatment
We verify your insurance and give you the numbers in writing. Nothing gets scheduled until you have seen what it costs.
What to bring
A photo ID
Your dental insurance card, and your medical card if you have one
A list of any medications you take, including supplements
The name and phone number of your previous dentist, if you have one
For a child, their guardian or a signed consent if someone else is bringing them
Patient forms
Health history is completed with the practice directly, never through this website.
Prefer paper? Ask at the desk and we will hand you a clipboard — arriving fifteen minutes early is enough time.
Things people ask about before coming
If the dentist makes you anxious
Tell us. We slow down. Blankets, music, breaks whenever you want one — you are in control, and stopping is always allowed. If it has been years and you are embarrassed about it, we have heard it before and there will be no lecture.
Bringing a child
First visits are short and friendly — we count teeth, answer questions and keep it calm. Parents stay in the room. If your child is having a hard day we stop and try again rather than forcing it.
